The Eclectica Free Estriol assay for the Eclectica analyzers has been designed for the quantitative determination of Free Estriol (FE3) in human serum and heparinized plasma. The method can be used to measure free Estriol concentrations over the range of 0.1 to 40 ng/ml (0.35 to 138.88 nmol/l).

Estriol (Estra-1,3,5(10)-triene-3,16a, 17ß-triol) is a C18 steroid hormone with a molecular weight of 288.4 daltons. It is the principal circulatory estrogen hormone in the blood during pregnancy.

During pregnancy, estriol production is predominantly from the foetal/placental unit (1). It is released from the placental and is conjugated in the maternal liver as glucuronides and sulphates. It exists in the maternal blood unconjugated (8%) and in conjugated forms (92%).

Maternal serum unconjugated estriol levels increase rapidly following the first trimester from 1.2 ng/ml at 15 weeks to about 12 ng/ml at term. Estriol circulating in the blood has a short half-life of 20-30 minutes (2,3), and so variation in the foetal/placental unit should rapidly be reflected by changes in the maternal serum estriol levels. Measurement of serum unconjugated estriol can therefore be useful to monitor foetal conditions (4).

Consistently low levels of estriol throughout pregnancy, or a sudden drop of estriol levels in serial determination may be indicative of foetal distress (5) or placental failure. Chronically low estriol values may be caused by other factors unrelated to foetal distress or placental failure (6).

The Eclectica assays for the Eclectica analyzers consists of:

  3. Eclectica Immunoassay Common Reagents Kit: for general chemistry reaction.

  5. Eclectica Systemic Wash Solution: Wash Solution of Eclectica system.

In the Eclectica Free Estriol assay, a polyclonal anti-E3 antibody is used in an enzyme immunoassay system which incorporates magnetic solid phase separation.

All incubation times and reagent volumes are determined by an assay specific software protocol.

Fixed amounts of fluorescein-conjugated anti-Free Estriol antibody and a Estriol derivative conjugated to alkaline phosphatase are added to sample, control or standard. The reactants are incubated at 37°C for 18 minutes. During the incubation Free Estriol in the sample, control or standard competes with the derivative for binding to the anti-Free Estriol antibody.

At the end of the incubation period anti-fluorescein coupled to a magnetic solid phase is added in excess. This rapidly and specifically binds to the Free Estriol-antiserum complex and is sedimented in a magnetic field.

After aspirating the liquid phase and washing the solid phase, a solution of the enzyme substrate, phenolphthalein monophosphate, is added to the incubation cell and each cell incubated at 37°C for 42 minutes. After incubation the enzyme reaction is stopped by the addition of a reaction stop reagent and the intensity of the color developed is measured photometrically.

The intensity of the colour developed is inversely proportional, within the working range of the assay, to the concentration of Free Estriol in the sample. The concentration of Free Estriol in a patient sample or control is then determined by interpolation from the stored standard curve.
