The Eclectica DHEA-S assay for the Eclectica analyzers has been designed for the quantitative determination of DHEA-S in human serum and heparinized plasma. The method can be used to measure DHEA-S concentrations over the range of 0.05 to 10 µg/ml (0.13- 27.1 µmol/l) of DHEA-S.

Dehydroepiandrosterone sulphate (DHEA-S), a 19-carbon steroid, is the most abundant adrenal androgen in the circulation. Most of the circulating DHEA-S originates from either direct adrenal secretion (1, 2) or by peripheral sulphation of dehydroepiandrosterone (DHEA) secreted by the adrenal cortex (3). In males, some DHEA-S is secreted by the testes but in females the ovaries never secrete DHEA-S even in pathological disease states. Circulating levels of DHEA-S increase from about the seventh year of life, peak in the third decade and decrease gradually thereafter (4).

DHEA-S has a long half-life (5) and circulates at much higher levels than other androgens or related steroids. Unlike other adrenal steroids eg. cortisol, it does not exhibit diurnal variation (1) nor does it circulate bound to specific binding proteins as does testosterone (6). Serum DHEA-S therefore may serve as a specific and stable marker of adrenal androgen secretion (7).

Although DHEA-S is only weakly androgenic, it can be metabolised to more potent androgens such as androstenedione and testosterone. Consequently DHEA-S can indirectly be the cause of hirsutism or virilization. DHEA-S levels are often assayed together with testosterone in the investigation of these conditions (8) and one or other of these androgens is elevated in many patients with hirsutism (8, 9).

Serum DHEA-S levels are increased in patients with Cushing's disease (7) but not in patients with adrenal adenomas. Therefore measurement of serum DHEA-S can be useful in the differential diagnosis of Cushing's syndrome (7).

Measurement of serum DHEA-S is also useful in the following endocrine conditions:

  1. Infertility - in hirsute, amenorrhoic women the suppression of DHEA-S can restore menstruation and fertility (10, 11).
  2. Congential adrenal hyperplasia - very high DHEA-S levels can be obtained which cause virilization (9).
  3. Precocious puberty.
  4. Adrenal hypo- and hyperfunction - dynamic function tests are used to investigate these conditions. In adrenal hypofunction DHEA-S levels do not respond to ACTH administration; in adrenal hyperfunction administration of dexamethasone can suppress DHEA-S levels for 30-90 days (12).

The Eclectica assays for the Eclectica analyzers consists of:

In the Eclectica DHEA-S assay, a rabbit anti-DHEA-S antibody is used in an enzyme immunoassay system which incorporates magnetic solid phase separation. All incubation times and reagent volumes are determined by an assay specific software protocol.

Fixed amounts of fluorescein-conjugated anti-DHEA-S antibody and a DHEA-S derivative conjugated to alkaline phosphatase are added to a sample, control, or standard. The reactants are incubated at 37°C. During the incubation DHEA-S in the sample,control or calibrators competes with the derivative for binding to the anti-DHEA-S antibody.

At the end of the incubation period anti-fluorescein coupled to a magnetic solid phase is added in excess. This rapidly and specifically binds to the DHEA-S antibody complex and is sedimented in a magnetic field.

After aspirating the liquid phase and washing the solid phase, a solution of the enzyme substrate, phenolphthalein monophosphate is added to the incubation cell and the each cell incubated at 37°C. After incubation the enzyme reaction is stopped by the addition of a stop reagent and the intensity of the color developed is measured photometrically. The intensity of the color developed is inversely proportional, within the working range of the assay, to the concentration of DHEA-S in the sample. The concentration of DHEA-S in a patient sample or control is then determined by interpolation from a stored standard curve.
