The Eclectica Anti-hTg assay for the Eclectica analyzers has been designed for the quantitative determination of Thyroglobulin autoantibodies in human serum and heparinized plasma. The method can be used to measure Anti-hTg concentrations up to 6250 IU/ml of Anti-hTg. Primary calibration is against the WHO 1st International Reference Preparation (WHO 1st IRP 65/93) for Anti-hTg.

Thyroglobulin (hTg), the prothyroid globulin, is a high molecular weight (660 kDA) soluble glycoprotein made up of two identical subunits. hTg is present with a high degree of heterogeneity due to differences in post translational modifications (glycosylation, iodination, sulfation etc). During the process of thyroid hormone synthesis and release, hTg is polymerized and degraded. Consequently, the immunologic structure of hTg is extremely complex. The characteristics of hTg preparations may vary widely depending on the starting human thyroid tissue and the purification process used. This is the first clue to explain why hTgAb assays, as well as hTg assays are so difficult to standardize.

There is some debate over the clinical utility of serum hTgAb measurement for assessing the presence of thyroid autoimmunity. In iodine deficient areas however, hTgAb is believed to be useful for detecting Autoimmune Thyroid Disease (AITD), especially for patients with a nodular goitre. hTgAb measurements are also useful for monitoring iodine therapy for endemic goitre, since iodinated hTg molecules are more immunogenic.

Serum hTgAb testing is primarily used as an adjunct test when serum hTg measurements are requested. The clinical utility of hTgAb measurements in sera from Differentiated Thyroid Cancer (DTC) is two fold. First, sensitive and specific hTgAb screening of sera in these cancer patients is necessary, because even low antibody concentrations can interfere with the hTg measurements made by most hTg methods. Second serial hTgAb measurements themselves may serve as a surrogate tumour markers test for hTgAb positive patients in whom hTg testing may be unreliable. Specifically, hTgAb positive patients who are rendered disease-free typically become hTgAb-negative within 1-4 years. In contrast, patients who have persistent disease after treatment retain detectable hTgAb concentrations. A rise in the hTgAb level is often the first indication of recurrence in such patients.

The Eclectica assays for the Eclectica analyzers consists of:

In the Eclectica Anti-hTg assay two hTg-conjugates are used in an immunoenzymetric assay system which incorporates magnetic solid phase separation. All incubation times and reagent volumes are determined by an assay specific software protocol.

Fixed amounts of fluorescein-conjugated hTg and a hTg-conjugated to alkaline phosphatase are added to a sample, control, or standard. The reactants are incubated at 37°C. During the incubation fluorescein-hTg conjugate, Anti-hTg and an alkaline phosphatase hTg conjugate forming a sandwich.

At the end of the incubation period anti-fluorescein coupled to a magnetic solid phase is added in excess. This rapidly and specifically binds to the Anti-hTg complex and is sedimented in a magnetic field.

After aspirating the liquid phase and washing the solid phase, a solution of the enzyme substrate, phenolphthalein monophosphate is added to the incubation cell and the each cell incubated at 37°C. After incubation the enzyme reaction is stopped by the addition of a stop reagent and the intensity of the color developed is measured photometrically. The intensity of the color developed is directly proportional, within the working range of the assay, to the concentration of Anti-hTg in the sample. The concentration of Anti-hTg in a patient sample or control is then determined by interpolation from a stored standard curve.
